CHCC provides behavioral health and access to social services because this can play an important role in one’s overall health. CCHC has Licensed Clinical Social Workers (LCSWs) available at each site. CCHC’s primary care providers work closely with the behavioral health providers to ensure a coordinated plan of care. Services are intended to meet the patient where they are in their ability to manage their health.

Referral Information
Comprehensive Community Health Centers are in various cities in Los Angeles but work independently of each other. Each center has a behavioral health and social services department on site. In order to utilize the behavioral health services, you must contact your health insurance and have your Primary Care Provider (PCP) reassigned to Comprehensive Community Health Centers, Glendale. After having your PCP reassigned, please call (818)-265-2264 to speak with a receptionist and schedule an appointment. During the initial appointment, the medical provider will conduct an evaluation before referring to the behavioral health department. Space and availability with the behavioral health services varies.
